A Community Health Needs Assessment (CHNA) is a structured process used to identify and evaluate the most pressing health issues within a specific community. It combines data analysis (such as health outcomes and demographics) with direct input from community members and stakeholders to highlight key challenges, disparities, and gaps in services.
